Temporary - Disabled Person Parking Permit Placards

Temporary parking permit placards are available to any individual with temporary mobility impairment or those who are temporarily sight impaired. The disability must be certified by one of the following:

  • Physician/physician assistant licensed to practice under Chapters 458, 459 or 460, Florida Statutes or similarly licensed by another state.
  • Osteopathic Physician
  • Podiatric Physician
  • Chiropractor
  • Physicians who practice medicine in a military medical facility, state hospital or federal prison must complete in detail the "Physician's Statement of Certification" section on form HSMV 83039, indicating the facility and the address.
  • Adjudication Office of the United States Department of Veterans Affairs or the US Veterans Administration.

In addition to further requirements:

  • Must have either a current Florida Driver License or a Florida Identification Card issued by the Florida Division of Driver Licenses.
  • Must submit a completed application.
  • Temporary disabled person parking permit placards may not exceed 6 months from the date of issuance.
  • The Application for Disabled Parking Permit (form HSMV 83039) may be obtained at theTax Collector’s office.
  • The fee is $15.00.

Disabled Person Parking Permit Placards – Long Term

Long term parking permit placards are available to any individual with long-term impairment, permanent mobility impairment or those who are certified legally blind. The disability must be certified by one of the following:

  • Physicians/physician assistants licensed to practice under Chapters 458, 459 or 460, Florida Statutes or similarly licensed by another state.
  • Osteopathic Physician
  • Podiatric Physician
  • Chiropractor
  • Optometrists (for sight only)
  • Physicians who practice medicine in a military medical facility, state hospital or federal prison must complete in detail the "Physician's Statement of Certification" section on form HSMV 83039, indicating the facility and the address.
  • Adjudication Office of the United States Department of Veterans Affairs or the US Veterans Administration.

In addition to further requirements:

  • Florida Statutes, provides that a person who claims a disabled parking permit has been lost or stolen must submit an application and a certificate of disability (form HSMV 83039) issued within the last 12 months.
  • An advanced registered nurse practitioner licensed under Chapter 464 in a facility operated by the United States Department of Veterans Affairs under the protocol of a licensed physician.
  • Long term parking permit placards are issued for four years.
  • Initial and renewal issuances of a long term disabled person parking permit placard requires a current application form with the completed certification. Only application forms revised after December 1996 or later may be accepted.
  • Must have either a current Florida Driver License or a Florida Identification Card issued by the Florida Division of Driver Licenses.
  • Must submit a completed application.
  • Long-term disabled person parking permit placards are issued for a period of four years and expires on the disabled person’s birthday.
  • The Application for Disabled Parking Permit (form HSMV 83039) may be obtained at the Tax Collector’s office.
  • There is no charge for the issuance of Long Term Disabled Person Parking Permit placards.

Disabled Person License Plate

Florida Statutes provide for the issuance of a wheelchair symbol plate to any owner or lessee of a motor vehicle who qualifies for a disabled parking permit. This qualification can be obtained by accurately completing the Application for a Disabled, Disabled Veteran or Motorcycle International Wheelchair Symbol License Plate (HSMV 830007) or contacting the Tax Collector’s office.
